Project Title: American Promise Video
Project Description: Submission of existing and/or user generated content to American Promise for a film about the organization to be used to broaden awareness and increase support.
Release and Consent: By completing this form and submitting videos or other media, I hereby grant American Promise, and its legal representatives, the irrevocable right and permission to use and publish the videos I have submitted, including any stills, graphics, or audio recordings related thereto, for the purposes of an American Promise video project, in any and all forms of media, now known or hereafter invented.
I confirm that the submitted video is my own original work and does not infringe on the rights of any third parties. I also confirm that all subjects appearing in the video have given their consent for their image, likeness, and voice to be used in the project.
Rights and Ownership: I understand that American Promise will have the right to edit, modify, add to, subtract from, and otherwise alter the video, and to combine it with other images, sounds, and recordings for the purposes of the project.
I also understand that I retain ownership of the original video, but I grant American promise the non-exclusive, worldwide rights in perpetuity to use the video for the project without any further approval required from me..
Acknowledgment: I hereby release and discharge American Promise from any and all claims and demands arising out of or in connection with the use of the videos, including, without limitation, any claims for defamation or invasion of privacy.
Agreement: I have read and fully understand the terms of this release form. This agreement shall be binding upon me and my heirs, legal representatives, and assigns.
